Instructions
- This page provides three blank forms: Import Declaration, Export Declaration and English Version Declaration. Click the tabs above to switch between them.
- Light-yellow areas are fillable. Click "Print / Save as PDF" to output in A4 landscape format, or download the Excel template to fill in offline.
- The goods table reserves 6 rows by default. For more items, click "Add Goods Row" below the form; item numbers are renumbered automatically.
- GACC Announcement No. 30 of 2024 has removed "Inspection and Quarantine Acceptance Authority", "Port Inspection and Quarantine Authority" and "Certificate Issuing Authority"; "Destination Inspection and Quarantine Authority" was changed to "Destination Customs", and "Inspection and Quarantine Name" was changed to "Supervision Category Name".
- For gross or net weight less than 1 kg, the value should be accurate to 2 decimal places according to the latest 2024 requirements.
- Actual declarations shall be subject to the current requirements of China International Trade "Single Window" and the competent customs. This template is for reference only.
